Hip Replacement following Failed Internal Fixation of Intertrochanteric Fracture of Femur

Abstract
Purpose: To analyze the clinical and radiological results of hip arthroplasty following failed internal fixation of intertrochanteric fracture of the femur.

Materials and Methods: Sixteen patients of hip arthroplasty after failed internal fixation of intertrochanteric fracture were followed for more than 24 months postoperatively. Fractures were classified by Evans type and Singh index. The
adequacy of reduction after internal fixation was analyzed. Clinically, the amount of blood loss and Harris hip score (HHS) were measured. The stability of cup and stem after arthroplasty was assessed using various radiological indices.

Results: Fractures were fixed wit 14 compression hip screws in 14 hips, with Ender nail in 1 hip and with gamma nail in 1. The fixation failed after a mean duration of 8.5 months. All fractures were unstable. Singh index was less than
grade 3 in 11 cases (68.7%). The reduction was inddequate in 12 cases (75%). At the latest follow-up, mean HHS was increased from preoperative 36.1 points to 73.4 points. There was no incidence of femoral fracture or perforation during operation. All stems were stable at the latest follow-up. There was no cup loosening except 1 case of vertical migration of bipolar head.

Conclusion: Satisfactory results were obtained with salvage arthroplasty for failed intertrochanteric fixation.
